Report: Hospital staff don't report 86% of patient harm events

All 189 hospitals surveyed in a report rely on incident reporting tools, but their staff failed to report 86% of patient adverse events, in part because they view certain events as unreportable. The report, released by the HHS Office of Inspector General, proposed that the CMS and Agency for Healthcare Research and Quality come up with a list of potentially reportable harm events as well as offer training and guidance to hospitals.
