WakeMed begins Patient Navigator program as part of ACC pilot

WakeMed Health & Hospitals, part of an American College of Cardiology pilot project to reduce readmissions, is using its grant money to launch a Patient Navigator program that assigns hospital staff to help patients after discharge. Heart failure and heart attack patients are targeted for the intervention, which aims to provide patients with medical information in easy-to-understand terms.
