Lack of testing is cited for misaligned valves at Calif. nuclear plant

Federal regulators have identified the lack of proper testing for the misalignment of two safety valves at the Diablo Canyon nuclear plant in California. Modifications conducted during a refueling outage in 2008 prevented the valves from being opened remotely, but the problem was not detected because the necessary testing was not performed, the Nuclear Regulatory Commission said.
