Violations of fall-protection standards have ranked third in the top 10 list of most cited rules in 2013 from the Occupational Safety and Health Administration in Oregon, according to this article. The agency issued 431 citations on fall protection, including such violations as the improper use of ladders and a lack of safety equipment against fall hazards. "We need to change the culture that accepts rule violations and occasional penalties. Unfortunately, that culture still exists on some job sites," said Michael Wood, administrator of Oregon OSHA.
