Insurers, other stakeholders urge addition of 5th health plan tier

Some health insurers, business groups and politicians are urging the Obama administration to create a fifth, lower tier of health insurance plan that would essentially halt the planned phaseout of coverage that does not meet current Affordable Care Act standards. The low-premium plans would include greater upfront costs for enrollees.
