Outcomes have improved for ambulatory heart failure patients but hospitalized patients still have high rates of post-discharge mortality and readmissions, an international team of researchers wrote in the Journal of the American College of Cardiology. The researchers said there is a need for global hospital-based registries to improve understanding of the HF patient population and to guide policy, research and treatment.
