Geisinger lowers readmissions with telemonitoring program

The Geisinger Monitoring Program uses an interactive voice response system and telemonitoring solutions from AMC Health to increase physician-patient interactions post-discharge and has lowered hospital readmissions by 44%. The program's success shows "that providing clinicians with the right data, at the right time, to drive the right interventions is critical to driving the best patient outcomes," said Jim Peters, a vice president of Geisinger Health System.
