Transition to medical home model has challenges, study finds

Physician practices that switch to a medical home model face challenges, such as establishing a teamwork mentality among staff and obtaining expensive technology needed to make the system work, according to a study in Health Affairs. Data from the first National Demonstration Project on medical homes found while the patient-centered approach to care improved quality and controlled costs, the transition was time-consuming and disruptive.
