Practices in Enhanced Primary Care initiative earn bonuses

Nine physician practices were awarded performance bonuses for improving patient care under the Enhanced Primary Care initiative sponsored by Capital District Physicians' Health Plan in Albany, N.Y. The program, which has lowered costs by reducing hospital admissions, encourages preventive care and early intervention. Rommel Tolentino of Community Care Physicians said the practice used its lump payment to hire a nurse to coordinate care and to set up an e-mail communication system for patients and physicians.
