Okla. Medicaid finds PCMH reduces hospital visits

Oklahoma's Medicaid program uses the patient-centered medical home model and pays participating physicians more for coordinating patient care. Study data show using the PCMH model has reduced hospital admissions, readmissions and emergency department visits in the state.
