Top 10 diseases health systems target to cut readmissions

A survey by the Healthcare Intelligence Network identified the top 10 conditions targeted for hospital readmission reductions. Heart failure topped the list with 89.6% of respondents saying their organization was working to cut HF readmissions, while 68.8% are focused on chronic obstructive pulmonary disease and 66.7% on pneumonia. Heart attack, cardiovascular disease and diabetes were also on the list.
