Senior managers have a crucial role in promoting and assessing safety practices in process industries, writes Graeme Ellis. Leadership buy-in, regular evaluation and allocation of sufficient resources are among six tasks Ellis lists. "I believe that ensuring a strong response to near misses is the most important of these. Leaders should require a thorough investigation of near misses, to find weaknesses in the overall process," he writes.
