Better access, coordination are goals for ACOs, medical homes

Consumers, health care providers and payers agree that key goals for accountable care organizations and medical homes include better access and care coordination to improve outcomes and lower costs, according to a new report by the Patient-Centered Primary Care Collaborative. Health information technology and provider payment reform, two other areas of consensus, are seen as tools that can ensure new delivery models succeed, the report said.
