Connecticut plan calls for more people in home-based care

A plan in Connecticut calls for improving patient transitions to home care as health care providers work to resolve issues such as poor showings for home care quality measures, high hospital readmission rates and low lengths of stay for hospice care. Tracy Wodatch of the Connecticut Association for Home Care and Hospice said people go back and forth between home care and hospital care before entering hospice.
