Safe STEP helps elderly transition from hospital to home

A new program to help the elderly transition safely from hospital to home emphasizes communication and teamwork among health care providers of varied disciplines. A study shows that three days after discharge, 88% of patients who participated in the Safe and Successful Transition of Elderly Patients program described their health as better than it was before their hospital stay, compared with 79% in a control group that did not have the Safe STEP services.
