IBM, Oklahoma medical school embark on "medical home" initiative

The University of Oklahoma's School of Community Medicine will partner with IBM to develop an IT-based model for coordinating patient-focused medicine in primary-care practices. The "medical home" framework will use portals and electronic health records for documentation and data sharing, as well as bolster evidence-based care via decision-support platforms. The project will involve 355 doctors and link information from EHR systems used by several groups, including hospitals and ambulances.
