Report: Federal inspectors botched their own safety inspections

Federal regulators who monitor the transport of hazardous materials failed to conduct safety reviews at their own laboratories for at least 10 years, according to a government watchdog agency. Labs used by the Pipeline and Hazardous Materials Safety Administration to test explosive materials were unable to provide records of their activities, and some had outsourced testing of explosives to unauthorized labs.
