Calif. health plan using TriZetto technology to manage chronic conditions

CareMore Health Plan aims to help its members better manage chronic and acute ailments by deploying a platform from TriZetto that uses clinical algorithms to design personalized health interventions, including compliance monitoring. The Clinical CareAdvance system will allow CareMore, a group that provides Medicare Advantage plans for 32,000 elderly patients, "to leave behind episodic care delivery" and meet the needs of its clients in real time, an executive said.
