Exchange engages patients in diabetes telehealth program

Western New York's HEALTHeLINK clinical information exchange is having early success with a pilot program that uses mobile health monitoring to identify diabetes patients at risk of hospitalization. The initiative has enrolled 116 participants and is supported by nurses who visit and train patients in the use of a telemonitoring tool for secure transmission of data such as fasting glucose and heart rate.
