Medicare plans chronic care programs to cut health care claims

Certain chronic illnesses such as diabetes and asthma are responsible for most Medicare spending, but Medicare officials hope to control those expenditures and improve care by creating 10 disease management programs intended to prevent crises that require hospitalization. Coordinated care pilot projects involving 15,000 to 30,000 patients will be managed by private companies who will be required to demonstrate at least 5% decreases in claims.
