Hospital benefits from chronic care management technology

A bidirectional chronic condition management system is helping providers at Denver Health Medical Center better manage care for chronically ill patients and lower the costs related to hospital readmissions for patients with diabetes. The technology "may be an effective method to provide care for chronic conditions beyond the traditional clinic setting," the hospital's Director of Medical Informatics Dr. Andy Steele said.
