Hospitals bring back the house call

Some hospitals have begun sending teams of health care providers and case managers to discharged patients' homes in an effort to provide better treatment and lower costs. In-home care is often less expensive than in-hospital care, and the teams can help patients with medication adherence and give caregivers a break while working to prevent readmissions. Experts say accountable care organizations have the flexibility needed for this approach.
