Team-based care for seniors tied to better outcomes

The team-based Mobile Acute Care of the Elderly model was associated with lower rates of adverse outcomes and shorter hospital stays for seniors with acute illness compared with usual care, according to a study in JAMA Internal Medicine. However, researchers said that the patient-centered program did not reduce 30-day readmission rates or boost functional status compared with standard care.
