Investigators say poor supervision contributed to overpass collapse

Investigators looking into last year's fatal bridge overpass collapse in Ontario found the contractors and engineers on the project lacked the proper experience and the project was inadequately supervised. Experts were not present to oversee the installation of steel bars used to reinforce the roadway, and evidence indicates incorrect installation of the bars may have been the main reason for the collapse, which killed five people.
