Study: Home care program reduces admissions, length of stay

A program that managed care for home-bound patients with advanced illnesses led to a 37% drop in hospital admissions and 1.5-day reduction in length of hospital stay when admitted, despite illness progression, researchers from North Shore-LIJ Health System in Great Neck, N.Y., reported at the annual meeting of the Society of General Internal Medicine. Researcher K. Sandy Balwan, M.D., said a big challenge was getting real-time information on patient hospital admissions, noting that in 2012, the advanced illness management multidisciplinary team was unaware of about 25% of admissions.
