N.J. alliance uses PCMH model to coordinate care

Integrated Medicine Alliance, a Monmouth County, N.J., primary care practice composed of 40 clinicians at eight offices and three urgent-care clinics, operates on the patient-centered medical home model using a single EHR platform. Physical therapists, dietitians, urgent-care providers and other specialists work with primary care physicians to avoid fragmented care and help patients make healthy choices, says CEO William Febus. Payer support for care coordination allows IMA to hire needed staff, Febus says.
