Virginia-based Old Dominion Tree and Lawn Care Specialists is facing $185,500 in proposed civil penalties after a 14-year-old worker was killed in a wood-chipper accident in November. Frank Anthony Gornik was pulled into the machine while using a shovel to drop debris into the wood-chipper's hopper. The Virginia Department of Labor and Industry's Occupational Safety and Health Compliance department later issued two citations, and the company has 15 days to appeal the fine or pay the penalty.
