Hospitalists jump to nursing homes to provide transition care

Hospitalists are moving to skilled nursing facilities to help patients transition from hospital care, physicians said at the annual meeting of the Society of Hospital Medicine. While there is not yet a nursing home medical specialty or a core set of competencies for transitionalists, Dr. Kerry Weiner of hospitalist company IPC said physicians who make the jump will have to learn additional skills involving palliative care and the treatment of dementia, depression and pain.
