3 ways EHRs can help providers transition to PCMH model

Providers seeking to have their practices designated as patient-centered medical homes by the National Committee for Quality Assurance must first demonstrate effective use of EHRs. To get closer to becoming a PCMH, provider practices should be able to use EHRs to glean data on diseases, gather information on quality indicators and poll patients to find out if they are getting the level of care they need.
