AHRQ asks for input on revised patient-safety reporting proposal

The Agency for Healthcare Research and Quality has issued a notice updating its proposal to create a prototype system that consumers can use to report medical adverse events. The revised plan, which outlines the agency's proposed data-gathering techniques such as online and telephone reporting as well as provider follow-up surveys, is up for comment for 30 days.
