Study evaluates outcome-based transformation model for primary care

Researchers at the University of North Carolina at Chapel Hill developed and tested an outcomes-based transformation model for primary care practices. Their report in the Annals of Family Medicine said of the three trends that emerged over two years of study, the most prominent was that transformed practices lead to broad-based improvements.
