CMS plans IT pay-for-performance pilot for chronic disease care

CMS has proposed a three-year, pay-for-performance pilot program involving four practices that will use information technology to help manage care for chronic disease patients. The agency is seeking comments on the demonstration, which would include practices with at least 50 Medicare beneficiaries in Arkansas, California, Massachusetts and Utah.
