Ambulatory care nurses find opportunity in changing environment

A health care shift toward wellness and prevention is creating opportunities for ambulatory care nurses to work in team-based delivery models and create care coordination programs. Registered nurse Kathy Mertens and colleagues at Harborview Medical Center in Seattle are testing a primary care model in which an RN care manager works with a nurse practitioner and others, and a care team creates weekly action plans and works with patients in person or by phone.
