N.J. initiative aims to curb heart failure-related readmissions

More than 50 hospitals, nursing homes and other health care organizations in New Jersey are collaborating to curb hospital readmissions related to heart failure in the state. The initiative includes data sharing as well as informational sessions for patients and providers on the factors leading to heart failure readmissions and how to reduce them.
