Two medical systems in the Lynchburg, Va., area joined forces earlier this year to create the Collaborative Health Partners of Virginia, and the organization has applied for certification as a patient-centered medical home. Central Virginia Family Physicians comprises two urgent care facilities and eight primary care practices; Medical Associates of Central Virginia is a standalone practice. Patients will have access to all participating physicians, hospitalists and specialists, and some are already reaping the benefits of the model. One woman who has diabetes has seen a 100-point drop in her blood-sugar level.
