N.J. health group to use HIT for better hypertension, diabetes control

Trenton Health Team, which is a collaboration of two local hospitals, has received $415,000 from New Jersey's Health Department to develop a program that uses health IT tools to better monitor the health of people with hypertension and diabetes, which are substantial public health issues in the area. Goals include lower rates of hospitalization, fewer patients with diabetes whose blood sugar is high and better control of hypertension.
