Agency advises certified EHR use in chronic care management

The CMS has recommended that chronic care management providers use certified EHR systems and other health IT platforms that feature a care plan that can be accessed and shared by all involved medical personnel. The agency also stressed that such providers "need to establish reliable flows of information from emergency departments, hospitals and providers of post-acute care services to track their CCM patients receiving care in those settings." The CMS is seeking to make the use of certified EHRs mandatory for 2015 for chronic care providers.
