Report: Epoxy flaw, lack of inspection to blame for Big Dig collapse

A final report from the National Transportation Safety Board on the probable cause of the collapse of a Big Dig tunnel in Boston last summer reportedly focuses largely on the improper use of epoxy in a ceiling support system. The flaw was never discovered due to a lack of follow-up inspection, according to the Boston Herald.
