Ore. program connects frequent ED users with physicians

A pilot project to reduce Medicaid costs in Oregon uses care coordinators to help patients who are frequent users of hospital emergency departments find regular physicians. The federal government has given Oregon $2 billion and five years to determine whether the program can reduce medical inflation by 2%.
