Use of "medical home" care model grows

The federal government and insurers are using programs that increase payments or reward primary care doctors for creating a "medical home" model in which they coordinate a patient's entire range of care, as well as offer longer office hours, same-day appointments and electronic medical records. The efforts are seen as a way to reduce health care spending through fewer hospitalizations, ER visits and diseases.
