Care teams with NPs help reduce rehospitalizations

A study at a Minneapolis nursing home found using special care teams reduced hospital readmissions among older, short-term patients by 20%. The care teams included a nurse practitioner, physician, pharmacist and sometimes a dentist, who all worked to help prevent complications that likely would send a patient back to the hospital.
