Nurses have key role in strategies to prevent HF readmissions

Six hospital strategies could reduce heart failure readmissions by 2% and save more than $100 million, Yale University researchers wrote in Circulation: Cardiovascular Quality and Outcomes. One recommendation is to have nurses supervise patient medication plans. The other strategies are scheduling follow-ups before discharge, collaborating with other hospitals, sending discharge information to primary care providers, telling patients about all screening results received after discharge and creating partnerships with community care providers.
