When companies go off the rails, it's usually because of "organization drift" rather than "evil managers," says Robert Hurley of Fordham University. Creating a culture of ethical behavior requires strong leadership and rigorous scrutiny of workers further down the ladder. "Tone at the top is always good, but if behavior in the middle is unethical, there will be trust violations," Hurley says.
