MGMA survey: Physician practices want to be medical homes

Almost 70% of 341 physician practices surveyed by the Medical Group Management Association said they were interested in becoming or already were in the process of becoming a patient-centered medical home. Established PCMHs said some of the biggest challenges in switching to the practice model included setting up care coordination agreements with referral physicians, finances and coordinating care for high-risk patients.
