PCMH is part of movement toward care coordination codes

CMS has proposed new evaluation-and-management codes for care coordination services for patients with multiple chronic conditions, and House legislation also calls for such codes and mentions the National Committee for Quality Assurance's medical home and patient-centered specialty practice recognition programs as proof of a practice's ability to provide such care. "We are particularly pleased the draft includes expedited recognition of patient-centered medical homes as an approved alternative payment model for medical practices," said American Academy of Family Physicians President Dr. Jeffrey Cain.
