Program tied to lower 30-day readmission rates, study finds

The Project Better Outcomes for Older Adults through Safe Transitions was tied to lower 30-day rehospitalization rates, a study in the Journal of Hospital Medicine found. Data showed units in the BOOST program reduced readmission rates from 14.7% ahead of implementation to 12.7% at one year, compared with 14.0% and 14.1%, respectively, in the control group.
