Alzheimer's initiatives benefit patients, family, caregivers

Initiatives outlined at the Alzheimer's Association International Conference to benefit patients, family and caregivers include a program among Boston hospitals that refers them to the Massachusetts/New Hampshire chapter of the Alzheimer's Association for assessment by a care consultant. A Tufts Health Plan project uses nurse managers and primary care providers to track how caregivers are faring, and a joint effort by the city of San Francisco and Kaiser Permanente is working to improve education and support for caregivers and families.
