Health "coaches" offered to chronically ill in pilot Medicare program

Chronically ill Medicare patients in central Florida soon will be offered access to health coaches who will work with them on making medical decisions and preventive care under a three-year pilot program dubbed Medicare Health Support. The program will be free to participants and will feature hot lines to counsel patients on issues ranging from what questions they should ask their health care provider to how to set up living wills.
