Study finds improvements needed in hospital discharges

New study data showed hospitals can place patients in danger due to poor discharge procedures. The study found few hospitals include in discharge summaries patients who had test results that were pending and would come in after they left. Some discharge summaries also didn't mention test results that would require changes in a patient's treatment plan.
