Ohio ACO, senior care provider join forces

A partnership has been formed between Senior Independence, a division of Ohio Presbyterian Retirement Services, and Northwest Ohio ACO to use a Home to Stay care transition initiative to boost the health of the accountable care organization's beneficiaries. The program is expected to improve patient experiences and decrease readmissions. "Before Home to Stay, many seniors were left on their own -- some with dementia. Now, they're getting help with medications, being provided an evaluation of their home setting, learning which questions to ask at doctor appointments and creating lifesaving personal health records," said Senior Independence President Rich Boyson.
